Social Services Available at Rolling Hills

We’re Here to Make the Transition Easier

The Social Services Department of Rolling Hills is comprised of two Social Workers who will be able to assist you with finding the appropriate placement for your loved one whether they are coming from a hospital, home, assisted living facility, or transferring from another nursing home. They help our short-term rehab patients with discharge planning to the community, and can assist with setting up home care, meals on wheels, lifeline and order equipment. They also make referrals to ADRC if alternate placement is needed. In addition, the Social Services Department is also able to assist with applying for Medical Assistance and helps answering any financial concerns you may have.
